Skilled Nursing Facility
What is a Skilled Nursing Facility (SNF)?
A Skilled Nursing Facility (SNF) is a clinical care provider offering 24-hour licensed nursing care. An SNF primarily provides services for residents requiring medical and nursing care and/or therapy services for rehabilitation. A licensed physician supervises each patient’s care and a nurse or other medical professional is always on-site. Other medical professionals such as occupational or physical therapists are also available. Care typically includes: physical therapy, occupational and speech rehabilitation, intravenous therapy, post-surgical stabilization, pulmonary management, and wound care. Also, SNF’s provide assistance with administration of medications and aid in a variety of daily living needs including dressing, bathing, walking, and eating.
What is the difference between a Skilled Nursing Facility (SNF) and an Assisted Living Facility?
An Assisted Living Facility offers housekeeping services and medication management, provides meals, and helps with Activities of Daily Living. Assisted Living Facilities do not offer skilled nursing care or rehabilitation.
How can I determine if my loved one needs a Skilled Nursing Facility (SNF)?
If your loved one needs 24-hour nursing care and/or is unable to perform daily care activities, administer medication, prepare meals, etc., an SNF will provide the care they likely need.
What payment plans are accepted at Senior Care Centers’ SNF’s?
Medicare, Medicaid, Private Pay, and Insurance Plans.
How do I know if my loved one is ready for an SNF?

- Requires daily assistance with eating, dressing, and self-care
- Forgets to take medication or overmedicates
- Behaves in ways that could be harmful (to self or others)
- Wanders away from home or experiences frequent signs of memory loss
